Creative Pathways - Contract

      This contract is a legally binding document. Please read through it carefully and in its entirety.
 
This daycare is a State and City licensed daycare home. I meet with safety, health and nutritional standards. Taking care of children is my choice of professions. I enjoy caring for kids and watching them grow and become attached to the other children in my care.

I have child development experience through other daycares I have worked with, as well as classes that I have taken. I am also trained in CPR and first aid. I have experience with children with hearing loss, autism, seizure disorders, asthma, and stomach problems such as severe acid reflux.

I depend on this income as you depend on yours and would prefer to have children that are here on a consistent basis. This also helps me to provide a substitute on occasions when it is necessary for me to be gone for short periods of time, or if I become sick. Please understand that you will need to have a back-up sitter in case of emergency and I am not able to watch your children.

Days and Hours: Monday through Friday, open at 6:30 am and close at 5:30 pm.
If something occurs, please notify me if you are not going to be here or if your child will be late.

Meals: I provide breakfast, lunch and snacks at no extra charge to you. Breakfast is from 7:00 - 8:00, morning snack is from 9:00 - 9:15, lunch is from 11:45 - 12:30, afternoon snack is from 3:00 - 4:00.
 
 If your child will not be here during those times please make sure they have had their meals.
 
Please Do NOT let your child bring ANY food unless it is an extra snack on his or her birthday and there is enough for all the children. No gum or hard candy. These will be taken away.

Food Allergies: Any food allergies that your child may have, I need to know first thing, for their own safety. If I am not told, I cannot be held liable for any information that is not disclosed to me.

Safety: My first concern is for the safety and well being of the children. I have locked cabinets, safety locks, socket covers, safety gates, smoke detectors/fire extinguishers, and hold fire drills with the kids. The yard is fenced, and we keep the gate locked at all times. This is for the safety of your child (ren) as well as other kids under my care. The kids are not allowed outside the fence or outside my front door unless they are accompanied by an adult. Please do not have your child (ren) go outside the door (not even to your vehicle) without you.
 
It is impossible to assure that no child will ever be injured in childcare; however, I do attempt to keep the children as safe as possible. If any accident or emergency does occur, you will need to cover the expense.

Illness: State Law states that you provide documentation of your child's immunization with-in 14 days of attendance (see Daycare Immunization Law). I will not service sick or contagious children. Please use good judgment in bringing your child to daycare. Keep them home if they are throwing up or have a high fever. If your child (ren) becomes sick during the day, you will be expected to pick them up.
 
Medication: No medication will be administered without consent (see registration form). Furnish me with a note for the prescribed medication on the times to be given. Please do not leave medication in the diaper bag or on the counter. Make sure I get it.

Supplies: (that can be left here). Diapers, baby wipes, potty training diapers, blanket for nap, Tylenol, toothbrush, change of clothes. Please mark your child's name on all supplies. Bring child appropriately dressed for the day in play clothes. Bring extra clothes for a change in weather.

Discipline: I maintain certain rules in my daycare. If discipline becomes a problem, I will schedule a conference with parents and try to work on what could be causing the destructive behavior.
My discipline policy and the way that I handle discipline problems is posted on my wall. If for some reason the child’s disruptive behavior persists, the child may be un-enrolled after three documented disciplinary problems.

Vacations and Time-out: If you are going to be out for any reason, please let me know ahead of time. If you are going to be out for vacation, let me know at least four weeks in advance. Because this is my sole source of income I will charge you for vacation the first year your child is with my daycare. Every year after that, I will not charge you for one week of vacation time. If you take more than one week vacation, you will pay the remainder of days whether your child (ren) is/are here or not.
 
 I am closed most legal holidays: New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day - If Independence Day, Christmas or Thanksgiving fall on a Thursday, I will also be closed the following Friday, and clients with full time status will be charged at the normal weekly rate. I am also closed on Good Friday.

Rates: I have weekly rates, not daily. You are charged even if your child is not here. However, you are allotted 4 free sick days a year. You are also charged for 3 legal holidays that I am closed, Christmas, Independence Day, and Thanksgiving Day.
 
Please read the section below on full and part time designations and reference the calendar for more detailed information.

Extra Charges: I hope to have field trips for the children when school is not in session. Parents will provide for the expenses. (These will not be expensive outings, just like ice cream, etc.)

There is a charge of $5.00 every five minutes you are late picking up your child, to be paid that Friday with normal payment. A $15.00 per day fee will be charged for late payments.
 
A $35.00 fee will be charged on returned checks and only cash will be accepted thereafter, and you will pay all my fees if it causes me to have bounced checks.
 
 Accounts that are 30 days late will be sent to collection. There will be a yearly review of all charges and a small rate increase if costs exceed profit margins. Any child may be immediately and permanently un-enrolled for failure to pay in accordance with this contract as long as the client is given a week of notice of such due payment.

Payment: Payment is due on Fridays for the following week (see Registration Form). I require that you pay in advance. If you are not going to be here, please pay on Thursday. I also require a two-week notice prior to removal of your child (three week notice if more than one child) from daycare for which payment will be made.

I am looking forward to a good working relationship with all my children and their parents. I have done childcare for many years, and cannot see myself doing anything else. I have never encountered anything such as a bad relationship with any parents, and I hope if you have any questions, you feel free to ask and I will do my best to work with you and make your child's experience in my child care a happy one.

Full and Part Time Status:

When you enter into a contract for service with us, we will ask you to designate your child or children as being either full or part time. Please read below and understand the important differences between each classification.
 

Full Time Status:
• Children who enroll with full time status are guaranteed their enrollment with two exceptions for a period of one year from the signing of the contract with said status.

1. They may be un-enrolled for failure to pay in advance or any other breach of contract.
2. They may be un-enrolled for repeated (more than three instances) and documented disciplinary problems at the discretion of the owners/director.

• Children who are enrolled with full time status are expected to pay the current full time rate every week except when granted discounts for recognized closed holidays, which exclude such payment.

Children who are enrolled with full time status are eligible for full service up to five days a week.

• Parents must grant two weeks notice before switching their child or children from full to part time status. They will be expected to pay at the full time rate for the duration of these two weeks. Enrollment may be terminated at this point if no part-time enrollment slots are available.
Part Time Status:
• Children who enroll with part time status are not guaranteed their enrollment at Creative Pathways, and may be un-enrolled with four weeks notice in favor of new clients seeking full time status.
• Children who enroll with part time status may be permanently un-enrolled from all services at any time with four weeks notice.
• Children who enroll with part time status are eligible for three days of full service at the part time price listed below.

Current part time rate: $92 a week for three days of service

Additional days of service may be granted to clients with part time enrollment status at the following increased rates.

Four days at part time status: $115 a week (rate effective until Aug. 31, 2008)
Five days at part time status: $130 a week (rate effective until Aug. 31, 2008)
